Self-Pay and Out-of-Network
Current fees for services can be obtained by calling or emailing the office. Once scheduled, cost of services will be invoiced using Square at time of booking with the expectation that payment will be made upon receipt and prior to the scheduled appointment. Cost of services will be refunded if appointment is cancelled or rescheduled with at least 1 business day notice prior to the appointment. Exceptions to this policy will be made at the discretion of the practice manager. Payment forms accepted include all major credit cards only. If you cancel with at least 1 business day notice, payments will be returned in the form they were made and may take 2-7 business days before reflected on your statement. All patients are expected to place a valid credit card on file. Those who may have out-of-network (OON) benefits can ask for a monthly superbill from the office so that you may submit and receive reimbursement from the insurance company under your policy benefits. Patient is responsible for submission to the insurance carrier. To see if your policy offers OON benefits, we recommend calling your insurance company directly or looking at your policy online.
Insurance and In-Network
We accept most United Healthcare, Oxford, UMR, Oscar, Aetna, Cigna, and BCBS plans through Alma Community Network, LLC , and Headway in the states of NY, AZ, and FL not including Medicare or Medicaid plans. Cost of services depend on your individual plan; some patients may be required to meet deductibles before benefits start, whereas most plans have a set copay for all visits regardless of type. Both Alma and Headway perform insurance verification through their system and provide support related to billing and claims. Insurance verification may take at least 1 business day after obtaining all necessary data through intake forms. Please fill out all forms sent by the office in entirety to ensure proper and timely insurance verification. After verification, you will receive an email with coverage details and anticipated costs. Copays are collected by the Alma team after completion of your appointment via Square invoice in the form of a credit card or secured payment method, and Headway will automatically take the funds from the method of payment on file. Clients are expected to pay for copays immediately upon receipt of an invoice from Alma and upon completion of an appointment through Headway. For clients with outstanding copays, this may impact the ability to schedule future appointments. All patients are expected to place a valid credit card on file. No-shows and cancellations with less than 1 business day notice may be subject up to a $100 fee depending on visit type, even if rescheduled and will be determined at the discretion of the practice manager.

No-shows may impact the provider's ethical ability to continue care and patients will be given notification of such after two no-shows.
Client-Provider Relationship
The expectation is that after an initial evaluation, the client will return within 2-4 weeks if started on new medications. Follow-ups for refills and medication management are expected to return monthly. A patient can request to make an appointment at anytime to see the provider and will be placed on the schedule within a time period that the provider determines is safe and appropriate. Clients who have a pattern of repeat cancellations and have one or more no-shows and are being prescribed medications may be given adequate legal notice on finding another provider based on ethical considerations. The provider is not obligated to reschedule or see these clients once they have been given notice of a list of recommended providers.
Services We Do Not Cover
Due to the nature of some psychiatric conditions that can require comprehensive services and are also considered medical conditions, we cannot see patients who seek assistance for treatment of substance use disorders including for the purpose of rehabilitation, maintenance, or withdrawal. It is recommended that patients who require specialized services seek appropriate outpatient substance use programs such as Bridge Back to Life . Additionally, the prescriber does not prescribe ketamine or psychedelics for mental health reasons, but can recommend centers that offer these medications such as mindbloom.com. In the State of New York, marijuana may be prescribed for medical and psychiatric conditions, but the prescriber is not registered with this program at this time. We do not offer worker's compensation services or fill out disability claims. We can refer out for requests related to ESA (emotional support animals). Additionally, we do not offer forensic exams or psychiatric evaluations for legal matters including but not limited to court cases, immigration services, civil suits, criminal investigations, fitness for duty, child custody, divorce proceedings.
About The Providers
Our clinicians are board-certified psychiatric nurse practitioners in the State of New York and/or Florida. By law, this profession is authorized to diagnosis and treat most psychiatric disorders listed in the DSM-V. Upon request, the provider will furnish licensure number and any other credentials for the purposes or proof of valid qualifications and that license is in good standing. Please visit the New York State Office of Professions website for more information regarding this or the Florida Department of Health for verification of licensure of any of our clinicians.
COVID-19
The public health emergency has ended as of May 11, 2023, and the requirement for in-person visits related to controlled substances and prescribing, see Ryan Haight Act, will be required per federal and state law starting December 2024. Please make arrangements to be seen in person at our office if required over the course of treatment with our office and we will also be reaching out in relation to how this may impact delivery of care. Controlled Substances and Prescriptions
Our provider understands the importance of stimulant use for attention-deficit disorders, hypnotics for sleep disorders, and benzodiazepines for treatment of panic disorder and severe anxiety states. Generally speaking, the DEA has identified these substances as having determined levels of risks that may lead to physical and psychological dependence. Please read more about this here. What this means is that patients may end up becoming addicted to a substance when that was not their initial intention for the simple purpose of trying to reduce psychiatric symptoms. For this reason, the provider will discuss indication, risks, and benefits for the use of such substances and will write prescriptions in a way that is safe and effective and will require appropriate monitoring. In some cases, if you are taking these substances on a daily basis, the recommendation may be to safely taper off these medications until they are appropriate to use on an as needed basis. Therapy has been effective in assisting with decreasing the use of controlled substances. The State of New York does not allow more than a 30-day supply of controlled substances and all prescriptions are required to be sent electronically. Once a prescription is filled, it will require a 23-day period to pass until the next prescription can be picked up, although insurances may not cover early refills before the 30-day period and pharmacy or state policy may prohibit early dispensing.
Pre-Authorizations for Medications and Costs
Patients should not be impacted when it comes to medication refills and cost of treatment but it happens sometimes for those who rely on insurance to cover benefits. Insurance plans have control over what medication and the forms in which they chose to cover. If they deny coverage of a specific form (oral, transdermal, etc.) or type (generic vs. brand), which can happen in some cases, the office will attempt to contact the insurance company to justify the psychiatric and medical need for such. The office may take up to 5-7 business days to complete this process with the insurance company and provide proof of final decision. Pre-authorizations are sometimes approved for a limited period of time and are usually good for one year, but will need to be renewed periodically. Some pharmacies such as Capsule have a pre-authorization department who are capable of completing this process within 1-2 days. Telehealth Platform
The provider currently sees patients through a secure, HIPAA-compliant portal called Doxy.me. See contact for waiting room link. All that is necessary to join a visit is an electronic device that has a camera and a microphone. Please connect to a stable internet connection. To receive telehealth services, you must be physically located in the State of New York or Florida at the time of the visit according to state laws.
Referral to Other Providers and Diagnostic Testing
In certain situations, it may be medically necessary to refer patients to their primary care physician or specialist if certain bloodwork or physical monitoring is required while on medications, before starting certain medications, or as a result of the nature of the mental health diagnosis. Examples of this are routine lab work, heart rate and blood pressure readings, baseline or routine electrocardiograms (ECG/EKG), and blood therapeutic levels. This will be dependent on your medical and psychiatric history, prescribed medications, and lifestyle choices. In many cases, it can be helpful to screen for a physical cause of psychiatric symptoms or vice versa. If the provider becomes aware of certain symptoms that exist and the recommendation is to be referred to an outside provider, you will be given a referral letter to assist with continuity of care. You may be asked to sign a release form by both offices so that exchange of information can take place. If patients are unable to follow-up, it could impact the ability to start or continue certain medications and treatment options within the practice.
